10. Affiliation/Organization: Input the name of your affiliation or institution, no more than 50 characters, use abbreviation if necessary

15. Postal Code: input the post code in accordance with your address

21. Email: input your email address, which is available to receive further meeting information
22. Country of Citizenship: select the country of your citizenship
23. Requited Housing: please select “yes” only when you register for an all inclusive package
24. Roommates Given Name/Family Name: Student all inclusive package includes a shared room. If you’d like to share the room with someone specific (he/she also registers for student all inclusive package), please input his/her name. Otherwise, please just leave it blank. Filling a roommate name does not mean making a registration for him/her.
25. Special Request: input your other special request on meals or other aspects of the meeting
26. Meeting Price: please select the registration package in accordance with your selected position and your payment currency
*All the packages cover meeting registration, tea breaks, lunches and dinners during the meeting, and other meeting social activities.
*CNY Packages are only available for domestic participants in China who will pay by wire transfer. For international participants out of China, please register USD packages.
*USD Packages will be issued an electronic invoice by Cold Spring Harbor Laboratory in USA. CNY Packages will be issued an official Chinese invoice by Cold Spring Harbor Asia in China.
27. Check In /Check Out Date: Check-in and check-out dates will appear automatically when you select an all inclusive package. The 4-day lodaging can’t be booked partially. But you could add extra days for your early arrival or late departure.
28. Payment Method: select your payment method in accordance with your registration package
*For registration packages in USD, please select “Credit Card” or “Check to Follow” as payment method; for registration packages in CNY, please select “Wire Transfer to CSH-Asia” as payment method. An instruction of wire transfer will be included in your registration confirmation email once the online registration is successfully submitted
29. Cardholder Name: input cardholder’s name printed on the payment card
30. Credit Card #: input your card number WITHOUT any spaces in between
31. Expiration Date (mmyy): input the 4 digits of the expiration date printed on the payment card
32. Pre – payment: input the amount (number only) for your prepayment, from $200 to full amount
*A minimum payment of $200 is required at registration and will be processed shortly after the online registration. Full payment is required before the due date announced on our webpage. If you authorize a partial payment only (at least $200), your card will automatically be charged the balance due about 4 weeks prior to the meetings. Please contact us ahead of time if you wish to pay the balance with another card or via check.
*Credit cards that are not accepted for payment will incur a service fee for every additional attempt. Please check with your credit card provider to ensure funds are available to cover the amount authorized.
